Bad audio, sound effects stop after a few successful plays

As others have reported, my sound effects stop playing after a few successful plays for each. This seems new since late March / early April 2016.

My system:
- Windows 10 64-bit, version 1511, build 10586.164
- NVIDIA High Definition Audio
- Realtek High Definition Audio

Things I have attempted (with restarts between each):
1. Change system audio quality from 24/48000, to 24/44100, and also 16/44100
2. Disable all other playback devices (including HDMI)
3. Run audio troubleshooter (which btw, sets quality back to 24/48000)
4. Under Enhancements, disable all sound effects
5. Disable/enable primary playback device
6. Unplug all other audio equipment (webcam, speakers - I use headphones)

This didn't help, the problem persists.

I went out on a limb and did a system restore to a point prior to Windows 10 build 10586.164. It worked! No more audio problem.

well, considering that you have a realtek, and windows 10, I had kinda of same issue, the solution was force this driver to install :

"0006-64bit_Win7_Win8_Win81_Win10_R279" available from realtek webpage, but, you need to see if your sound card is on the supported list

Had same audio problem as poster. Updated drivers and restarted computer. Fixed.

I'm guessing this is the issue where your intro vids aren't playing either, and double ESC gets you into the menu?

Disabling audio in the BIOS, rerunning FA, then rebooting and enabling it again works for me.

In my case the problem starts when disabling in-game music to listen to a CD or whatever in the background. Seems DX or the sound driver is getting confused with audio prioritizing related to FA and keeps the broken settings in the registry. A driver reinstall also does the trick because it resets the sound setup. As will a system restore, but that's a pita.

I have the latest realtek drivers but I can still reproduce the issue at will so the fix is not in the driver itself, but the act of reinstalling it. In my case, it seems that disabling/re-enabling the onboard audio in the bios forces some sort of PnP refresh of the DX settings, and the game starts working again,

I have Win10, no idea if this issue occurs on previous versions.

This happened twice in my win7 x32 but solved after reboot.

Yes this is a fairly old problem and it's fixed by rebooting

No. They want you to install Win 10 so that they can get your data.
